STX1B Variant-Specific Synaptic Dysfunction Drives Network Hyperexcitability in Human iPSC-derived Neurons
Listed in NCBI GEO
Pathogenic variants in the STX1B gene, encoding the presynaptic SNARE protein syntaxin-1B, are associated with a spectrum of epilepsy syndromes ranging from mild febrile seizures to severe developmental and epileptic encephalopathies.
Description
To elucidate variant-specific disease mechanisms, we generated human induced pluripotent stem cell (iPSC)–derived excitatory neurons from patients carrying two distinct STX1B variants: a de novo missense variant (p.G226R) linked to severe epilepsy, and a complex insertion–deletion (p.K45delinsRCMIE; p.L46M) associated with a mild phenotype.
Using single-cell patch-clamp, multi-electrode array (MEA) recordings, and bulk RNA sequencing, we identified diverse synaptic dysfunctions that converge on a shared hyperexcitable network phenotype. The G226R variant exhibited impaired vesicle replenishment during sustained activity and increased spontaneous neurotransmission, while the InDel variant caused reduced syntaxin-1B protein levels accompanied by compensatory synaptic upregulation.

Transcriptomic profiling revealed overlapping and variant-specific gene expression changes affecting synaptic signaling, cytoskeletal organization, and vesicle trafficking. Importantly, all human neuronal cultures were co-cultured with mouse astrocytes serving as feeder layers. As a consequence, the corresponding FASTQ files contain a substantial fraction of mouse-derived reads.
These were rigorously filtered out prior to downstream analysis to retain only human reads; however, users accessing or re-analyzing these data should be aware of this mixed human-mouse origin of the raw sequencing files. Together, these data demonstrate that distinct STX1B variants induce convergent network hyperexcitability through differential synaptic and molecular mechanisms, providing mechanistic insight into the pathophysiology of STX1B-related epilepsies.
